1/17/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home Ashiyah. Upon arrival to the home, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her afternoon. Ashiyah stated to QMHP-C that her afternoon was going well. QMHP-C discussed with Ashiyah that this session was going to continue to focus on her ISP goals. Ms. Hodge reported that she is having trouble with âgirlsâ wanting to come to her house and fight her and she is not about that because as her mother, âI am with the shitsâ with behaviors lasting 20 minutes in duration. 2. Behavior: Ashiyah provided positive eye contact and participation during the therapeutic activity. Ashiyah struggled with identifying negative and positive ways of managing anger as evidenced by moments of complete silence with behaviors lasting up to 20 minutes in duration. 1.Intervention: QMHP-C inquired about Ashiyah emotional well-being. QMHP-C engaged Ashiyah in a handout on anger management. The handout educated Ashiyah on recognizing triggers and learning positive ways to channel that anger. QMHP-C allowed Ashiyah to complete daily entry in her journal and processed Ashiyahs thoughts and feelings expressed. Next, QMHP-C allowed Ashiyah to identify thoughts that triggered her anger and assisted Ashiyah in identifying appropriate positive responses. QMHP-C assisted Ashiyah in defining expectations and clarifying her responsibilities and the importance of following the rules especially with following curfew, displaying respectful communication, not talking back. 2. Intervention: QMHP-C allowed Ashiyah to identify negative consequences of not following the rules and the danger that stems from her behaviors. QMHP-C engaged Ashiyah in open-dialogue for her to verbalize acceptable and unacceptable behaviors. QMHP-C also discussed recognizing negative peer interactions and how her group of friends engages in negative and disruptive behaviors. QMHP-C role-played with Ashiyah by allowing her to demonstrate several emotions such as (anger, frustration, sadness etc.) to facilitate an understanding of her own limitations of how much anger she can and cannot control. QMHP-C processed through open dialogue Ashiyah's response. QMHP-C then provided tentative examples for Ashiyah to follow to get a better understanding about her actions when displaying aggressive behaviors. 1. Response: Ashiyah was receptive to QMHP-C's suggestions for today's session. Ashiyah indicated that she really never recognized that her group of peers has been a negative influence and has causes her to stay in trouble by getting suspended and doing in-school suspension. Ashiyah indicated that she struggles with aggression and displaying negative attitude as she seeks attention and acceptance from others. 2. Response: Ashiyah struggles with engaging in respectful behaviors as her mother has reported that she talks back and becomes oppositional when it pertains to not wanting to be in the home and not following through with curfew. Through completing the handout, Ashiyah was able to verbalize anger and what causes her triggers that feud the behavior.
